Your notice says SR-22, and the rules depend on the state that sent it. These are the SR-22 requirements by state: the form, how long it stays on file, the lowest cover allowed and the state's own fee.
SR-22 requirements by state, in one table
Each row comes from that state's DMV, statute or insurance department page. The link in the last column opens it. The state name opens our page for that state.

The insurer's filing fee is not in the table, because no state sets it. Allstate puts it at about $15 to $50.
Which form your state uses
Most states call it the SR-22. Georgia calls it the SR-22A. Florida and Virginia ask for the FR-44 after a DUI, with higher limits than the SR-22.
North Carolina uses the DL-123. Maryland and Delaware use an FR-19 certification. A few states give the insurer's proof another name, and it works the same way.

Who needs an SR-22
The state decides, and your court or DMV notice names the form. The usual reasons are a DUI or DWI, driving with no insurance, a crash with no insurance, or a suspension for points.
Texas lists a second conviction for no insurance among its reasons. If your notice does not name a form, the agent reads it with you on the call.
Who files the SR-22 certificate with the DMV
Your insurer files it, not you. You buy a policy with the filing attached, and the company sends the certificate to the state.
Most filings now go in electronically. Wisconsin says electronic filings usually reach the record in one to two working days. The steps are on how to get an SR-22.
How long the requirement lasts
Three years is the rule in 26 states. Texas counts two years from the conviction or judgment date, not the filing date. Kansas, North Dakota and Ohio say one year.
Your own end date depends on the date your state counts from. The SR-22 end date calculator works it out from your state and that date.
Texas SR-22 requirements
Texas wants the SR-22 kept for two years from the conviction or judgment. The policy needs at least 30/60/25 in liability cover, and reinstatement costs $100.
The full rules are on the Texas SR-22 page. Other states drivers ask about most: Florida, Virginia and North Carolina.
States with no SR-22
Four states never ask an insurer to file proof. Each state's own page says why.
- New Jersey has no SR-22 or other insurer-filed proof of financial responsibility: L.1979, c.169 repealed the proof-for-the-future sections (N.J.S.A. 39:6-40, 39:6-41, 39:6-43 to 39:6-47) because every owner must already carry insurance. The state's page.
- New York has no SR-22 or other insurer-filed proof of financial responsibility: Vehicle and Traffic Law section 346 ended that filing in 1957, and the DMV relies on New York-issued insurance reported to it instead. The state's page.
- Oklahoma has no SR-22 or other insurer-filed proof of financial responsibility: Laws 2009, c. 62 replaced the certificate-of-insurance option in 47 O.S. 7-320 (and repealed the certificate sections 7-321 and 7-322), so proof is now a security verification form (insurance card), a deposit of money or securities, or self-insurance. The state's page.
- West Virginia has no SR-22 or other insurer-filed proof of financial responsibility: the Legislature repealed the certificate sections of its proof-for-the-future law (W. Va. Code 17D-4-10, 17D-4-11 in 1990; 17D-4-15 to 17D-4-19 in 2006), and the DMV now takes current proof of West Virginia liability insurance instead. The state's page.
